The Opportunity (EOI)

We’re seeking candidates that are empathetic, excellent communicators with 2-3 years of aged care/NDIS experience.

 

What You’ll Do

1.    Task Processing and Administration

  • Receive participant related requests and task handovers from the Senior Care Partner.
  • Convert instructions into clear actions and system updates.
  • Enter requests accurately into the relevant care or workflow system.
  • Process service bookings, cancellations, purchase, referrals, and invoice approval tasks.
  • Check care plan and budget alignment before actioning requests.
  • Refer unapproved, unclear, or out of scope items back for review.

 

2.    Service Co-ordination

  • Book approved participant services in line with care plans.
  • Liaise with rostering teams, providers, and internal stakeholders.
  • Confirm worker availability and participant preferences.
  • Help resolve scheduling issues, changes, missed services, and urgent updates.
  • Escalate unresolved gaps or service issues properly.

 

3.    Documentation and System Accuracy

  • Record all actions, changes, and communications accurately.
  • Maintain care notes, task logs, and communication records.
  • Ensure documentation is complete, current, and audit ready.
  • Support strong system accuracy across participants tasks and service updates.

 

4.    Care Plan and Budget Support

  • Action services only where they align with approved care plans and support arrangements.
  • Check that services are approved and funding is available before proceeding.
  • Identify and report budget risks, overutilisation, or unapproved requests.
  • Support the Senior Care Partner with accurate service and usage information.

 

5.    Participant and Provider Communication

  • Confirm bookings, changes, and cancellations with participants and providers.
  • Provide general nonclinical updates and service information.
  • Follow up providers on outstanding services or service delivery issues.
  • Maintain professional and timely communication at all times.

 

6.    Monitoring and Escalation

  • Track task completion, service progress, and outstanding actions.
  • Report delays, service gaps, and risks to the Senior Care Partner.
  • Escalate clinical issues, safeguarding concerns, service failures, and urgent matters.
  • Support reporting and workflow follow-up to keep services on track.

 

7.    Ongoing Care Support

  • Assist with monthly care management administration activities.
  • Support service monitoring and approved changes.
  • Help coordinate allied health, nursing, and community support services.
  • Support participant goals and independence focused service delivery.

 

8.    Review and Administrative Support

  • Prepare supporting documents for support plan or service reviews when required.
  • Compile current services, budge usage, and care notes.
  • Maintain trackers, reports, and workflow systems.
  • Support audit preparation and internal administration.

 

 

What You’ll Bring

  • 18 - 24+ months experience in administration or coordination in the healthcare environment.
  • Strong computer literacy and confidence using multiple systems.
  • High level organisation and time management skills.
  • Experience in administration, coordination, scheduling, rostering, healthcare or NDIS care support.
  • High attention to detail and data accuracy.
  • Strong written and verbal communication skills.
  • Ability to follow structured workflows and delegated tasks.
  • Ability to prioritise work and meet deadlines.
  • Display natural ability to focus, not get distracted/get bogged down in small details.
  • Sound judgement in identifying issues that need escalation.
  • Professional communication skills with participants, providers, and internal teams.

  • NDIS/healthcare administration.
  • Care coordination support.
  • Rostering or scheduling support.
