The Opportunity (EOI)

We’re seeking an experienced and compassionate Care Partner / Clinical Partner to support a growing Australian-based national aged care provider from our Fiji office.

You’ll coordinate and lead the delivery of high-quality care management services through the Support at Home Program. As the primary contact for participants, you’ll work with them, their families and health professionals to ensure services reflect assessed needs, personal preferences and available funding.

You’ll monitor care plans, budgets, service quality and outcomes, while providing day-to-day support to management and staff through the changing aged care landscape.

 

What You’ll Do

  1. Care Management and Service Coordination. Coordinate agreed care and services for participants receiving support in their homes. Work with support workers, the care management team and external health professionals to ensure consistent service delivery.
  2. Care Planning and Reviews. Develop and regularly review care plans with participants, families and health professionals, ensuring services reflect assessed needs, preferences and available funding.
  3. Participant and Family Communication. Act as the participant’s main contact for care management and service coordination, responding promptly to questions, concerns and changing needs.
  4. Rostering and Team Collaboration. Provide the rostering team with clear, current information about participant needs, preferences and service changes.
  5. Budget Monitoring. Monitor participant budgets, expenditure and service utilisation to identify and address potential surpluses or deficits.
  6. Documentation and Record Keeping. Maintain accurate, up-to-date participant records, care plans and service information.
  7. Risk Management and Compliance. Identify and respond to risks, incidents and safety concerns, escalating matters in line with company procedures. Comply with relevant aged care quality standards and organisational policies.
  8. Feedback and Continuous Improvement. Seek participant feedback, respond to service concerns and contribute to continuous improvement.

 

What You’ll Bring

  • At least two years’ experience with Home Care Packages / Support at Home and associated compliance requirements.
  • A diploma or tertiary qualification in Community Services, Aged Care or Case Management.
  • Knowledge of home care regulations, industry standards and best practices.
  • A sound understanding of aged care and NDIS compliance, including quality standards, WHS, infection prevention and control, and SIRS.
  • An understanding of the care needs of older people and people with disabilities.
  • Experience working collaboratively with a dedicated rostering or scheduling team.
  • Strong communication skills at all levels, with excellent interpersonal, leadership and time management skills.
  • A compassionate, adaptable approach and the confidence to work autonomously.

Experience in one or more of the following will be highly regarded

  • Previous experience in case management, clinical coordination or home care management.
  • An allied health or nursing background, such as an AHPRA-registered nurse, physiotherapist, or allied health professional. This is desirable, not essential.
